Please read and fill out the following form to begin lessons with HHK. We can't wait to see you at the ranch!

Rider Registration Packet

All pages of this form must be completed prior to your first lesson. If you have a primary or secondary diagnosis, a doctor’s release will be required due to PATH Intl. standards and the safety of the participant.

Returning Riders

If you are already a HHK participant and will be attending one of our camps or birthday parties, you only need to submit a new liability release.
